RELEASE OF LIABILITY - In consideration of being allowed to participate in any way with Young Kings Elite, its related events and activities, I acknowledge, appreciate, and agree that: 1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the Company immediately; and, 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS Young Kings Elite their officers, officials, agents and/or employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessor’s of premises used for the activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property associated with my presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasee’s, and, for myself, my child and our heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasee’s from any and all liabilities incident to my minor child's involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.

I hereby agree and give consent for the Young Kings Elite and/or partners to record, film, photograph, audiotape or videotape my child’s name, image, and performance (herein collectively referred to as “Recordings”). I hereby consent to the use of Recordings or my child’s name for advertising, promotional or commercial purposes of any kind at the sole discretion of Young Kings Elite including, but not limited to, the Young Kings Elite website, brochures, blogs, all social media platforms—including, but not limited to Facebook, Twitter, Instagram, YouTube, and/or for broadcasting on television or radio as determined by the Young Kings Elite. I understand that the Recordings may appear in electronic form on the Internet or in other publications outside of Young Kings Elite control. I agree that I will not hold the Young Kings Elite responsible for any harm that may arise from such unauthorized reproduction. I hereby waive any right to approve the use of this content now or in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of this content. I hereby release Young Kings Elite, its officers, employees, agents and its advertising and promotional agencies from any and all claims, demands, damages, actions, causes of action whatsoever which I, my heirs, executors, administrators may now or hereafter have against them or any part of them in respect to relating to the use of the Recordings, including but not limited to libel and invasion of privacy. I understand that this release is irrevocable by me so that Young Kings Elite may proceed in full reliance thereon. I hereby consent under the Freedom of Information Act (United States) to the collection, use and disclosure of my images and name in relation to the Recordings. By signing this form, I give this consent and release voluntarily on behalf of the individual named and I understand and agree to be bound by its contents.
